    • Psychiatric evaluation and medication management
    • Spravato® (Esketamine) therapy
    • Intramuscular (IM) Ketamine treatment
    • Transcranial Magnetic Stimulation (TMS)
    • Child, adolescent, and adult psychiatry

    • Arrive on time for scheduled appointments
    • Provide at least 24 hours notice for appointment cancellations when possible
    • Arrange for safe transportation after treatments that may cause sedation (such as Spravato® and Ketamine therapies)
    • Pay for services rendered in accordance with our payment policies
    • Treat our staff with respect and courtesy

    5. Appointment and Cancellation Policy

    We request that you provide at least 24 hours notice if you need to cancel or reschedule an appointment. Failure to provide adequate notice may result in a cancellation fee.

    Repeated no-shows or late cancellations may result in discharge from our practice or requirement of prepayment for future appointments.

    6. Payment Terms

    Payment for services is due at the time of service unless other arrangements have been made. We accept various forms of payment including credit cards, debit cards, and cash.

    Insurance

    We accept various insurance plans. It is your responsibility to verify that we are in-network with your insurance provider and to understand your coverage, including co-pays, deductibles, and any services that may not be covered.

    Self-Pay

    For patients without insurance or for services not covered by insurance, we offer self-pay options. Please contact our office for information about our self-pay rates.

This means you will receive information about your diagnosis, proposed treatment, potential risks and benefits, alternative treatments, and the consequences of refusing treatment.
